New Patient Registration
If you live in our practice area (See boundary map to right) and would like to register with us, please come and collect a New Patient Registration Pack from the surgery - the reception team will help with any questions that you may have. As part of this registration process it would be helpful if you could
provide personal identification such as a driving licence or passport as well as address identification such as a utility or council tax bill, please talk to us about this if you have any queries or questions.
Your medical details are held on the computer and are strictly confidential. You may request this information if you wish, and a charge is made for this service. Charges are also made for certain non-NHS examinations, reports etc. and more details can be obtained from a member of the Reception team.
